Places on seats may fill up and seats are given on a first-come-first-served basis so please fill out this form as soon as possible and no later than December 22, 2022.
Please let us know if you would like to pay by year or by month. Special service are given to students who pay by year. When payment by year is oversubscribed, in order to be fair, a lottery is held in January and the successful applicants are notified.
